It's located in the rear hatch, top right hand side near the screen, you have to pull the both trim panels off to access it, remove two connectors and loosen the screw holding it in place and slide it out.

That's definitely water damage.
Though so, as I suspected. When I removed the trim panel to check the connections between the screen and amplifier, Christmas time, there was a small foam pad just above the amplifier which was soaking wet, so much so it came off in my hand.
Water is obviously seeping past the tailgate seal and shorting out these amplifier's. Hope this helps others who encounter this problem.

Or even consider taking these panels off to see if there is a similar water issue BEFORE the the aerial amplifiers fail, water getting in where it shouldn't seems to be a SEAT and maybe to a much lesser extent Skoda trait, not really good enough when it costs money down the line ie outside warranty.

Edit:- has there been a build up of "stuff" like leaves or moss in the rain guttering at this area on the tail gate aperture?

Lost my DAB, ran my VCDS and got the following:

0013 - Antenna Amplifier 1 for Digital Radio
B1270 13 [009] - Open Circuit
Confirmed - Tested Since Memory Clear
Freeze Frame:
Fault Status: 00000001
Fault Priority: 6
Fault Frequency: 6
Reset counter: 180
Mileage:
Date: 2023.01.12
Time: 18:03:25
Voltage terminal 30: 14.7 V
Status of digital radio antenna 1-Antenna current: 0 mA
Status of digital radio antenna 1-Connection between radio and antenna amplifier: Open circuit
Status of digital radio antenna 1-Connection between antenna amplifier and antenna: Not available
Status of digital radio antenna 1-DAB_Bit_Error_Rate_FIC: 0.000000

Would guess this is a failure of the DAB Aerial Amplifer?? Next question how do I remove the boot trim?

Cheers

You have to be quiet firm with them, they are held on with spring fasteners, first remove the rear light access panels and then put both hands into the holes and pull down, then do the same with the trim around the rear screen. I found putting them back more of a pain than removing them.

Fitted my new 5F4035225B today, fixed the issues. Interestingly no sign of water inside the unit, slight trace on the outside on the metal part of the case:

Also noticed this seems to be a design issue, as with the trim off when you open the boot, the rainwater runs down the boot frame, very close to the DAB aerial. I followed advice and cling film and insulation tape have been placed on the replacement.

New part was from Seat Parts Direct £58.51, I picked it as they are close to me. They said they do sell a lot of these. I have got the top trim back, but I am missing a metal clip on the bottom section, any body know of the part numbers for these clips?

Thanks to @Wilto for the help with this(y)
