Ibiza S 1.4 TDI issue please advise

Hi all. Have an Ibiza 1.4 TDI 2009 that keeps going into limp mode.

With VAG COM I get the following 2 error codes.
Any advice on how I can get the issue sorted would be greatly appreciated.
