Coilover fitting question!

just wanted to double check before i go ahead and fit these,

the part show, does this replace my top mount or does my top mount attatch to it, or do i use just some parts from my current top mount for the below part

you know what, i posted this after just having a quick look, just had a proper look and relised it is the rear adjust, ill hide now

is it really essential though to take that plate out for the ball joint and even remove the disc and callipar
