Help needed with another limp mode problem

I've done a search and not found anything. My problem is that my lcr can go into limp mode whether I'm driving hard or like miss Daisy. I've been told its the d/v I've since change this with a new d/v. But its still going into limpmode. I'm wondering if the problem could be caused by either the N75/N249 or the actuator, as I've replaced nearly every vac hose.
